George’s Story: Preparing for a Safe Return Home
George, 83, is recovering in hospital after a fall. He wants to go home, but his daughter Donna is worried—George now needs a walker, his bathroom isn’t safe, and Donna’s not sure how much help her dad will need once he’s home.

When George gets home, we assess his mobility, daily routines, and home environment. We identify risks and recommend practical solutions—like grab bars, a raised toilet seat, or changes to how the home is set up. We also teach Donna how to help safely and connect the family with community supports for ongoing care.
With a clear plan in place, George’s transition home can be smoother, safer, and less stressful—for both him and Donna.
